The film begins with Balu, a strong and kindhearted man who helps the needy for minimal charges and falls in love with the beautiful Chitra. Meanwhile, tycoon Madhusudhan Rao becomes mentally unstable after an accident, leaving his benevolent wife Rohini to shoulder immense burdens. Her sly brother Dasu, once imprisoned, seeks vengeance by ruining her family. He manipulates her elder son-in-law Sagar through a prostitute, Vasantha, shattering his daughter Raji’s marriage, while his own children tempt Rohini’s younger two. At the same time, the malicious Jalarakshasudu threatens her peace. In desperation, Rohini turns to Balu, requesting him to pose as her elder son. Out of gratitude—since Rohini once helped him perform his mother’s last rites—Balu agrees. Though initially rejected by the family, his patience and determination win them over. Soon, Balu is shocked to recognize Madhusudhan Rao as the very man who once saved him from goons, an incident that caused his current condition. While Rohini insists it was just an accident, Balu notices she secretly withholds his medicine to maintain the façade of madness, suspecting deeper truths.
Step by step, Balu resolves family conflicts, reforms the misguided kin, and gradually earns their love. With proper treatment, he restores Madhusudhan Rao’s health, and the patriarch too accepts Balu as his elder son. When Balu presses Rohini for the truth, she reveals the past: Madhusudhan Rao had once witnessed and documented a crime committed by Jalarakshasudu, hiding the evidence to protect himself. Fearing for his life, Rohini kept him feigning insanity, convinced the criminal would kill him if he recovered. Balu vows to protect him. But Dasu betrays them, informing Jalarakshasudu, who abducts Madhusudhan Rao to retrieve the hidden evidence. In a gripping climax, Balu rescues him, defeats the villains, and restores peace. Finally, the movie ends on a joyous note with the marriage of Balu and Chitra.
